This is a new system to try to make it more straightforward for patients who need annual reviews of chronic diseases (particularly those who have more than one condition). We know that with busy lives it can be difficult to fit in appointments and also can be tricky to know which member of the team you need to see and when.
We are trying to make it more efficient for patients by having a series of appointments with the relevant members of the team in a short space of time. These appointments will review ALL of the conditions at the same time. Some of the conditions covered by this system include diabetes, high blood pressure, heart conditions and breathing problems such as asthma or chronic obstructive pulmonary disease (COPD).
To make it easy for patients to remember when their review is due, we have decided to plan for annual reviews to occur in the month of the patients birthday- hence the name “Birthday Clinics”
So how does it work?
- This year, we will send a letter several weeks prior to your Birthday.
- Phone for your appointment for the Birthday Clinic around the time of your Birthday.
- The first appointment will most likely be with a healthcare assistant who will perform tests such as blood pressure and blood tests. You will be asked to bring a urine sample with you.
- They will then arrange follow up appointments (shortly following this appointment) with the practice nurse or nurse practitioner for a more detailed review.
- This will be followed, if necessary, by a review with a GP after.

Routine and travel vaccinations for all ages are available during surgery times by appointment with our nurses. If you are travelling and require vaccinations you will need to fill in a form before your appointment – these are available from reception.
We run a flu vaccination clinic in late October please keep an eye out for dates.

A ‘one-stop shop’ clinic for patients on warfarin treatment.

A counselling service run by our in-house counsellor. Appointments are normally arranged after consultation with a health care professional.

Antenatal check ups performed by one of the community midwives.

Postnatal advice and support from a health visitor at The Hope Centre, Hereford Road
Time: Thursday, 10.30am -12.30pm
Run by a health visitor at The Hope Centre, Hereford Road. Provides advice on issues such as feeding, growth, development and behaviour.
Time: Every Thursday 10.30am – 12.30pm
The doctors perform minor operative procedures in the surgery including freezing warts, mole removal etc. Please discuss your condition with a doctor first.
